Functional Appliances
Functional appliances harness muscle and growth forces to modify the developing occlusion, classically to advance a retrognathic mandible in growing Class II patients.
Common Appliances
- Twin Block (Clark) — the most popular; two bite blocks posture the mandible forward; well tolerated and efficient.
- Activator (Andresen) — the classic/most studied tooth-borne functional appliance.
- Bionator — a slimmer modification of the activator.
- Frankel (FR) — tissue-borne; vestibular shields and lip pads remove muscle forces to allow expansion.
- Herbst — a fixed functional appliance (telescopic), good for non-compliant patients.
Mechanism
Most work primarily through mandibular advancement, producing dentoalveolar changes with a smaller skeletal component; best results are obtained during active growth.
